Covenant Between Candidates & Formators
Refer to Section 1 of the Candidate Handbook.
A covenant form should be custom prepared by the Formators for each first year Candidate prior to their first (September) Formation Meeting and signed by both the first year Candidate and the Candidate Formators. This covenant remains in effect throughout Candidacy. The first, second and third year Candidates and their Formators should then bring the signed covenant to every Formation Meeting.

Church Visit Report
Each month during your Candidate Formation, you are required to attend Mass or another liturgical function at three (3) locations where a deacon assists or presides. Third year are Candidates exempt from these church visits from October until Ordination.
The purpose of these visits is to help you to observe: the diversity in liturgical styles; the implementation of rubrics by priests and deacons; the effectiveness of the homily; the types of music ministries; the liturgical practices; the participation by other liturgical ministers and the congregation; and, any other aspects of the liturgy you found to be effective. The purpose of this requirement is to help you to grapple with the diversity of our diocese by observing our various liturgical practices. Concentrate on the positive effects of the liturgy you experienced or observed among the faithful in attendance. A brief report (no more than a paragraph or two) should be forwarded by e-mail to the Administrative Assistant for the Office of Diaconate Formation identifying the parish, the priest and/or deacon, the date and time.
A brief report for each visit must be submitted by e-mail to the Candidate Formators and the Office of Initial Diaconate Formation Initial.Formation@rcda.org. This form may be used after each visit, completed and attached to the e-mail.

Request Admission to the Ministry of Lector or Acolyte
Prior to June of his first year, the Candidate must request admission to the Ministry of Lector by submitting a letter to the Bishop through his Formators.
Prior to June of his second year, the Candidate must request admission to the Ministry of Acolyte by submitting a letter to the Bishop through his Formators.

Liturgy - Three Year Cycle of Liturgy Formation and Proficiency Check-List
A Three Year Cycle of Liturgy Formation is aviliable to help you plan for your Liturgy Training and proficiency during monthly Formation meetings.  Use this Liturgy Proficiency Check-List to track your progress in Liturgy training until all categories of training are marked either "Experience and Knowledge," or "Proficient."

Annual Formator Evaluation Tool for use by the Candidates
Prior to the April Formation meeting, each candidate should request and complete the Formator Evaluation Tool to evaluate their Primary Formators, the Homiletics Formator(s), and the Liturgy Formator(s).  At the April Formation meeting, Formators review the completed evaluations and return the evaluations to the Office of Diaconate Formation.

Semi-Annual Formator Evaluation by the Candidates
Prior to the January and May Candidate Formation weekends, each Candidate must use this form to evaluate their primary Candidate Formators and submit the evaluation at the meeting. Formators review and return the evaluations to the Office of Initial Diaconate Formation.

Semi-Annual Candidate Evaluation by their Formators
Prior to the January and May Candidate Formation weekends, the primary Candidate Formators must collaborate to draft an evaluation for each Candidate and discuss the evaluation at the meeting. Changes may be made to the evaluation after the discussion and before the evaluation is forwarded to the Office of Diaconate Formation.
